Crombie and Price now distribute the Special Foods Authority Prescriptions for Gluten Free foods. This is known as Hospital to Home Delivery and encompasses the 6 most southern District Health Boards and recently added to the scheme the Hutt Valley District Health Board.

Steps:

1. Upon receiving the prescription form faxed from the GP we will post (fax or email if you supply the details) a pro-forma invoice to you.
2. Check the Pro-forma invoice for correct quantities and products.
3. Payment. This can be made by any of the following methods:
          i. Cheque
          ii. Telephone Banking
          iii. Internet Bill Payment
          iv. Visa or Mastercard
4. Once payment is received we will dispatch by courier your Special Foods Prescription.
5. Delivery is free to nominated residences*
